In order for us to provide you with the best care, we set forth office policies so that we can maintain a high standard of care. We ask that you read this information carefully.
Please fill out the patient history information prior to your arrival for your appointment. If you do not have access to the internet, you should arrive 30 minutes prior to your appointment time so that the history sheet is completed before your appointed time. We need your recent mammogram /ultrasound /MRI films and the written radiology reports in order to provide the best care. In most cases, you will need to go to the imaging facility and pick up the actual films. In most cases, imaging facilities do not send films in the mail. If we do not have this information at the time of your appointment, you may not be seen.Please note that if your imaging studies were done at Virginia Hospital Center, you do not need to bring your films and reports, as we will be able to access them electronically.
Your appointment time is set aside for you. Kindly give 24 hours notice to cancel your appointment. We do maintain a “move-up” list to fill any cancellations with other patients who are waiting to be seen. If you do not cancel your appointment, and you do not come for your appointment, you may be assessed an additional fee when you make your next appointment. This fee will not be covered by your insurance. We strive to see each patient on time, and our appointment times reflect this. If you are more than 15 minutes late for your appointment, you may not be seen at that time. We do not send out reminders to let you know it is time for you to make an appointment. We do confirm appointments by telephone. There may be times when your appointment is moved due to urgent surgery. This is the nature of a surgical practice, and we ask that you understand this. Every effort will be made to reschedule your appointment as close to your original appointment time as possible.
Prescriptions and Forms
If you are given a prescription for any imaging study, and you misplace it, you will be charged 10 dollars per prescription in order to get another one. There is a 4 business day turn around for us to process lost prescription requests. We will not fax a prescription to the imaging facility on the day of your appointment if you arrive and find that you do not have your prescription. Many patients require multiple forms to be completed for their employer. There is a charge for all completed forms to be paid prior to submission of these forms.
Payment and Insurance
We participate with a number of insurance plans. It is your responsibility to understand your particular insurance coverage. Our relationship is with you, not your plan, and difficulties with coverage are between you and your carrier. Please call our office to confirm that we participate with your particular plan. Co-pays must be paid at the time of compliant service. If your plan requires a referral, we cannot see you if you do not have this referral. Insurance plans requiring referrals may not cover in-office sonography. In-office biopsies may need an additional referral, so these biopsies cannot generally be done on the same day as a new patient appointment. Referrals cannot be back-dated, and thus we cannot see you unless the referral is dated prior your office visit. This is not our rule but rather that of each managed care plan. If we do not participate with your plan, payment is expected at the time of service. We will provide you with all of the forms necessary for you to send to your insurance company so that you can be reimbursed directly from them. If you do not have insurance, payment is also expected at the time of service. Unpaid accounts may necessitate legal action, and further care will not be provided by our office.
